Somatostatina e/o Octreotide (analogo della somatostatina)

Utilizzo:
Per somministrazione sottocutanea utilizzando infusori temporizzati (modalità d'uso) che erogano la dose terapeutica (3 mg per il tetradecapeptide, 1 mg per l’analogo di sintesi octapeptide) nell’arco di 10 ore a partire dalle ore 20:00 circa. Sono disponibili anche confezioni a lento rilascio per uso intramuscolare dell’analogo sintetico octreotide che possono coprire fino ad un mese circa con un’unica iniezione intramuscolare (modalità d'uso).

Caratteristiche farmacologiche:
E’ un polipeptide (sostanza composta da amminoacidi) di 14 amminoacidi, che ha la proprietà specifica di inibire il principale fattore di crescita, il GH, che ha un ruolo fondamentale nell’insorgenza e proliferazione neoplastica. Svolge attività inibente anche su molti altri fattori di crescita che in misura significativa concorrono a determinare e diffondere la patologia neoplastica, quali prolattina, IGF, TGF alfa e beta, PDGF, EGF, VEGF. Numerosi analoghi di sintesi della somatostatina sono stati studiati e impiegati con successo in varie neoplasie. Fino dal 1988 negli USA la FDA ha approvato e registrato l’uso della somatostatina in diverse varietà tumorali. Attualmente un numero crescente di pubblicazioni sta confermando le proprietà antitumorali di questa sostanza in tutte le varietà neoplastiche con meccanismi d’azione sia diretti su recettori cellulari per la somatostatina, individuati nella cellula tumorale, che indiretti attraverso l’inibizione di quei fattori di crescita cui ormai è universalmente riconosciuto un ruolo primario nei tumori. Meccanismi d’azione sono antiproliferativo (inibisce la riproduzione di popolazioni cellulari neoplastiche), pro-apoptotico (induce una morte programmata delle cellule tumorali), antimetastatico (non consente la diffusione a distanza delle localizzazioni neoplastiche primitive. In queste azioni la somatostatina è sinergica e notevolmente potenziata dagli altri componenti dell’MDB, quali retinoidi, melatonina ecc.
